You can apply for a referral form, either by phone or from our website. Please see the link at the bottom of this page.
There are two parts to a referral form:
1) One part is to be completed by a health care professional e.g. GP, Psychiatrist, Occupational Therapist or Social Worker. They must also provide a Risk Assessment, CPA or Social Report.
2) The second part is to be completed by the applicant. However, the referrer will need to approve and sign the form.
Once we are in receipt of all the particulars and the referral has been accepted, we will then invite you in for an informal interview.
During the interview process you will be asked questions that relate to your previous work experiences and qualifications or skills gained. This helps us to establish how much experience you have and to identify what level of support and training you need.
If you have been successful, you will then be given a start date and placed on a four week trial. At this stage your are not committed, this is purely an opportunity for you to experience what the work involves and help you to decide if it is something you would like to pursue.
